What happens next?

We write down everything people have said at your Life Plan day. We might also use information that has already been collected like your support plans.You will get a copy to keep and give to people who are helping you move. Your Life Plan will help your Social Worker and the other people helping you, to start working together to make your future plans happen.

Having future direction

Having a Life Plan doesn’t mean that things will change quickly. This is because some things take a long time, like getting the right housing and support. Your Life Plan might be about things you want for your future, but if you have a Life Plan then people know what you want to happen because it has all been written down by you and the people close to you.
